At Hatzfeld we provide residential care for adults with varying needs such as Asperger’s, Schizophrenia or eating disorders to age-related illnesses or Dementia. A person-centred care plan is produced tailored to each individual’s specific needs.
Personalised care packages are agreed as part of the initial assessment process and are updated on an ongoing basis responding to the changing needs of the person. The needs and wishes of residents along with advocates and family members are included in the care plan process from the outset.
Robust and transparent quality assurance systems are in place and feedback from a variety of sources such as residents, family members, advocates and other health professionals are used to shape and continuously improve future service delivery.
Care Homes
Five homes from home
Each of our five homes offer a unique, safe and effective care setting allowing for appropriate support to be provided in a homely environment.
We have achieved great success working with people on a recovery pathway allowing them to return to living independently in the community following a period of rehabilitation in a therapeutic environment.
Sometimes reablement or recovery is not appropriate, but personalised support can be given to maintain dignity and independence as much as possible in a safe, homely environment for the longer term.
